Anonymously Report a Violation

Any individual (e.g. athletic department staff member, student-athlete, prospective student-athlete, university employee, representative of the University's athletic interests, member of the community) may report an alleged, rumored, or suspected violation. You may report a potential violation verbally or in writing. We are obligated to investigate all alleged, rumored, or suspected violations.

Please list all involved (including name of student athlete, prospect, coach, administrator, or representative of athletic interest) as well as the sport.

Please provide a description of the alleged violation, as detailed as possible (include the date the violation occurred if known).
